My father was discharged unbeknownst to me (his DPOA) and transported to his new facility with absolutely no communication from the social workers. After numerous voicemails and phone calls from myself, our placement officer, and the new facility director, the social worker failed to return or answer phone calls which resulted in my father being transported to his new care facility, which was not ready for him to move in, and resulted in him being sent back to ProMedica. If you value transparency about the care your loved one is receiving, updates from the medical staff, or any level of empathy and care, please do not look at placing your loved ones here.
Promedica Canton is a wonderful facility to work with.Staff is friendly and the facility is clean. Maria Kotsonas,the Comunity Relations Specialist at Promedica is a great person to work with.She goes above and beyond to provide Promedica with the best community resources available. I recommend Promedica and working with their team.
Heartland Health Care Center - Canton is a pleasure to work with. Their organization is full of hard working, wonderful staff, who are always looking at ways to get involved in the community. Canton Township - Club 55+ is very thankful to have such a valuable community partner. This past May we were in need for a mobility scooter for some of our participants to get around the center. Heartland stepped up to the plate and donate a beautiful brand new scooter for individuals with mobility issues, to use. Heartland also supports a number of our events/initiatives and are a wonderful source for resources for our participants. Such an amazing team to work with.
Its hard to pick an overall star rating for our experience. Sharon from OT was great--5 stars for therapy. She tried to get us into the facility and trained us to use the transfer equipment. She would get grandma washed up and dressed when the aids didnt have time. The nurse aids seemed kind and caring although understaffed. The nurse manager, LaTisha, would return my calls and keep me informed of my grandmas clinical status whenever she could. She was responsive to my concerns when they were brought to her attention. All of sub-acute care in this area is suffering from understaffing post-covid. I dont believe that the company cant do more to attract and retain staff. This facility seems to be one of the best around. Unfortunately, the whole system is broken. The portion of my grandmas care that I found really lacking was the absence of coordinated discharge planning. There was never a care meeting with the Heartland team and family. When my grandma was in Heartland Sarasota we were invited by zoom to a care meeting. There were some isolated conversations with social work. A home health nurse was set up as was PT. We were given a recommendation for self pay home health, but we are out of correct ostomy supplies and dont know how to get more. We never got the hoyer lift that was requested by therapy weeks ago. We ordered a ramp upon discharge, not realizing that it takes 2 weeks to actually get one. It could have been done much better with a collaborative approach.
I had a relative stay there for two weeks after hospitalization and they received excellent care. The nursing/staff was professional, compassionate and friendly. The PT/OT staff was excellent! The food was pretty good most of the time. We could not have had a better experience.
Our Mother was a patient at Heartland Canton for 30 days for rehabilitation. Mom received the best care during her stay here. The entire staff was top notch!Her PT and OT therapists were second to none!I live and work close to Heartland so I was able to pop in several times a day to observe and participate in treatments and activities with my Mom. What a blessing! It was so very comforting to know that our loved one was in such good hands. We were very pleased and grateful for our experience, and have highly recommended Heartland Canton to our Family and Friends.Thank You to the Canton Chamber of Commerce for recommending Heartland Canton to us! What a Gem!
I am a fairly new employee at Heartland of Canton, yet I consider to have spent long enough to be able to perceive the positive work atmosphere and the sense of responsibility that our facility is committed to.The caring culture is being impressed by our leader, who is a true role model for employees from all departments.The Business Office Manager role gives me the opportunity to meet with many of our patients and their loved ones. Thank you Heartland Hospice Team for partnering with us in taking care of our beautiful resident Ramona until the very end! You helped us keep our goal of No Covid cases in our facility while attending to the needs of our residents.The hospice nurses, the shower aid, and everyone at Heartland Hospice are simply angels. We could not have done it without their dedication, empathy, professionalism, and hard work. They pushed themselves hard through this pandemic to take care of those whos journey among us came to an end. It is a great partnership we will to continue between our facilities. I would recommend these folks to anyone who is currently searching for a hospice.
Hello,My name is Tea, and even though I haven’t been with the company for too long, I truly feel I find myself in a facility where my coworkers are truly passionate and caring individuals. On a daily basis, I see people going above and beyond to meet and exceed the needs of our patients and their families, as well as the community. As a Social Worker, these type of work ethics are very important to me. I recently became the Director of Social Work for Heartland of Canton, after starting out as a Social Worker, and I am honored to work with an excellent team of caring individuals. My favorite part of the company is our open-door policy, which makes me feel happy to know that I always have someone I can go to when challenging situations arise, receive or offer support, or simply take a minute and unwind. My stay at Heartland Canton was excellent, I came into the facility as a recovered covid patient that had been on a ventilator for around 145 days or so. Upon my admission I had a tracheostomy and feeding tube. The respiratory therapist Kim was awesome she was very tentative, caring, knowledgeable and patient with me. The nursing staff was awesome, Thomas, Liz, Mani, Matt and Edwin just to name a few that were my absolute favorites. The therapy staff Pia, Lisa, Annette did an amazing job, when I came in I had a previously broken ankle that happened 4 days prior to being put on a vent and it went unnoticed in the hospital.So to sum it all up, I went to HCR needing to learn to walk,talk,eat and breath correctly all on my own. Upon my d/c I WALKED out without oxygen, without a tracheostomy and no tube feed.If I could give them 20 stars I would, mainly because they made me feel safe and comfortable like they were my family in the midst of a pandemic when I couldn’t see my own family.
My mom was in need of rehabilitation after her stay in the hospital. They recommended Heartland of Canton. After looking at the reviews I was a little concerned.I drove to Heartland and was shown the facility by Andrea. She set my mind at ease and I found the facility clean and well lit. Most rooms are doubles with older but very useable furnishings.My mom was at heartland for 14 days. The staff was extremely pleasant and professional. I would like to thank nurse laura, and nursing supervisor Katrina for all the help they gave our family and my mom. I would also like to thank the entire staff for taking such good care of my mother.I would not hesitate to recommend Heartland of Canton to anyone looking to do rehabilitation.
Beautiful and well-kept facility with amazing staff! I am a home care representative and visit skilled nursing facilities/rehab centers daily. This facility, by far, is one of the nicest. All members of the staff are very attentive to the patients and the amenities are second to none. The rounding physicians are some of the top physicians in Wayne County. The therapy team is outstanding and always ensures patients are ready for a safe discharge home.This facility has been remodeled over the last few years and is always kept clean at all times. I love visiting Heartland Canton and working with the staff here. If you are deciding on a rehab facility for your loved one, choose Heartland Canton! You wont go wrong.
My mom was taken to Heartland for physical therapy & further care after a two week stay at the hospital. After her previous experience at another rehab facility, I was not looking forward to another rehab stay. Heartland has been amazing. She receives two in room physical therapy sessions every weekday, 24hr medical care, assistance with whatever she needs physically and mentally, good food, special treats, & she is comfortable. I receive multiple updates throughout the week on her progress & even what recreational activities they have to keep the patient’s spirits up. It really seems like the staff there really cares about each individual; going so far as to keep track of their patient’s likes & dislikes! It’s been an overall great experience & I would recommend Heartland to anyone who is in search of a rehabilitation facility.
My name is Van. I am a healthy 56 year old. However sometimes we have things go wrong. I had a hip replacement, and decided to go to a physical rehab facility. I chose Heartland Healthcare of Canton. It was a great decision! From the staff to the care team, it was exceptional! Very clean, very timely with care! My rehab team, was exceptional showing me ways to do my exercises to recover faster and without injury. And by the way, they have these new rehab suite s that are phenomenal. 5 stars across the board. Thank you Heartland!!❤❤
HORRIBLE place, dont let its fancy decor fool you. Do not leave your vulnerable loved ones here.... rehab was fine, the CNAs were TERRIBLE people. If your loved one is able to get up on their own and move around, it might be okay, but do NOT leave your bedridden loved one here. If I could leave NO stars I would.
I was at heartland for 22 days recovering from covid. I was treated very well during my rehab. All my needs were met and some workers went above and beyond to make my recovery possible. Special thanks to Mario, Lee and Lisa for their professional help and kindness. Ive never been in this type of setting so I didnt know what to expect but the great staff made me feel.comfortable while I recovered. Thank you all.
AWEFUL and disgusting. My father was suffering from brain cancer and was confused. ProMedica/Heartland had their transportation pick up my father after told MANY times to never transport my father anywhere. They dropped him off at Karmanos in Detroit, they left him there like a rat. He had no appointment out there either so why pick up and drop off? This was such a dangerous situation with my father and his confusion. I spoke to the admin and all he could say was, I have no words. This place is not only filthy, poor care and poor hygiene but also dangerous to our elders. This place deserves to be shut down!!!
Canton Heartland is a wonderful facility to work with. As a business development manager for a home healthcare company, I have worked with many people from different rehab facilities. Heartland is devoted to being a strong, patient-centered facility. David Liebrecht, the Business development specialist at Heartland is a great person to work with. He goes above and beyond in networking to provide Heartland with the best community resources available. Our collaboration helps provide the patient with the best outcome possible. Heartland Health Care Canton is a top-notch facility across the board. Not only is it clean and inviting for the residents, but its staffed with people who care about those residents as if they were their own family. A true diamond in the rough and a great place to trust with the care of your loved one.
Overall, I would rate the nursing care close to a 5 based on my phone interactions with the staff. The person I care for was there 8 days, and overall seemed satisfied with the food and care.He had one fall there the day before he was discharged trying to go to bathroom during the night. So, he came home with many contusions . Assume that was written up by O.T. who reported it to me.The only reason I gave the facility a 4 instead of a 5 is because they need to have better phone service. Over 8 days, I called to speak with a nurse several times, and the phone disconnected the call, before I could even reach a person. This happened not once, but several times. The day of discharge, I couldnt even reach the floor; the phone rang at least 10 times w/o being answered. If its an employee issue, they need to hire someone who can responsibly answer the phones; Its its the phone service that is at issue, it still needs addressing.In speaking with another visitor, he told me its always this way here!Someone in authority needs to correct this problem.
My mother stayed here during the last year of her life. She was went back and forth from hospitals to rehab facilities and she stayed in a number of different locations depending on availability. We were so impressed when she was admitted to Heartland Health Care Center - Canton. The staff was very friendly and professional and her private room seemed more like a luxury hotel! It gave us peace of mind to know she was getting great care during this difficulty time.
We took my brother to this facility with high hopes. He suffered a stroke and his kidneys shut down. He had lost the use of his right side, had troubles talking, couldnt walk (as a matter of fact, he couldnt roll over or sit up in bed) and needed dialysis 3 times a week.While he was there, he was taken to dialysis without fail and without ever involving us which was a great relief to us. The PT and OT and speech therapy that he got was nothing short of amazing. They worked him hard, made him believe in himself again and gave him the attitude adjustment that he needed to want to get better.He was able to walk out the front door upon his release. Hes not doing any marathons but considering how he was when he arrived, this was unbelievable. We cant thank everyone there enough for not only taking care of my brother but also for giving us back the man that wants to get stronger and better. Hes looking forward to traveling again and getting back to many of the things he could do before his stroke.I hope and pray that I will never need this kind of service but if I do, Ill be coming here.
I am a service coordinator at a senior independent living facility near Heartland Canton. We work closely with Heartland to produce educational seminars for the residents at my building. Heartland has been nothing but a pleasure to work with and they are always dedicated to relaying information to our residents about a variety of subjects dedicated to seniors. They are always pleasant and smiling when coming in our building and our residents enjoy coming down for the seminars and activities.In addition, numerous residents from our building have chosen Heartland Canton for their preferred rehab center. I have even had personal family members stay at Heartland Canton, where they enjoyed their stay and would chose this facility again if they had to. I rate Heartland Canton 5 stars!
I have tremendous gratitude for the Heartland Canton team. They offered excellent care and guidance for my mom while she was there. The staff was helpful to me over the phone as I live out of town. Knowing my mom was in good hands made me happy. I recommend Heartland Canton!
My sister had a major surgery and was in and out of the hospital for a month, she sustained a very large wound that needed packing twice a day, she became very weak, unable to sit up in bed. She went to Heartland rehab, the staff there was very caring and diligent with her treatment. PT and OT worked very hard to regain her strength. She is now walking with a walker and getting stronger every day. Her wounds are healing beautifully and she is ready for closure. Every time I would visit, the staff was very welcoming and supportive. Destiny, LPN and Tameka, Tech were always available to answer questions or to assist with any needs my sister might have had. Pat the case manager made time to discuss future placement plans, she was very attentive, and went above and beyond to meet my familys needs. All the staff there needs a huge pat on the back, they always greeted me with a smile. I never saw an employee that did not have a warm smile. To all the staff: nurses, assistants, housekeeping, food service, management and transportation thank you so much for your kindness and love. Your angels and I can tell how much you love your job. You guys are the best!!!!!! Keep up the awesome work.Thank you again.Susan
I have been employed by Heartland Canton for almost 5 years. I enjoy being able to meet all of our new patients and seeing the progress they make while they are here. I absolutely believe people choose our facility because our therapy team is the best. Our patients come in with goals to get back to their normal way of life, and we have the opportunity to help them achieve it. I have witnessed success time and time again. The amount of patients that use our services and choose us repeatedly speaks volumes. I am proud to be part of a team that respects, cares, and provides excellent care to all of our patients. I did a lot research, asked for referrals and visited several differentfacilities before deciding on Heartland Canton. My mother recently spent two weeks there after open heart surgery. The facility was clean, staff friendly and the food was very good. The most impressive part of her stay was her physical therapy. Her therapists were fantastic and she made great progress in her short stay there.
Re: Hip replacement recovery at Heartland Care Center - CantonI wasn’t sure I needed a rehab facility when I had my right hip replaced, but I wanted a successful recovery and it seemed like a good idea. Turns out I was right by starting my recovery at Heartland!In short, the physical therapist noticed I wasn’t using all my muscles during some exercises. Apparently, I stopped using some to compensate for the pain I was feeling prior to the surgery.Anyways, that problem might have gone unnoticed had I chose to recover on my own.- A private room- Personal attention from a caring physical therapist- A nice big facility to wander through- A friendly and understanding staff that was ready to helpI can’t say enough good things.One of the best decisions I made at the time. I recommend this facility to anyone!
I have had a very nice stay here. I am a long term care resident currently and everyone has been very caring and nice. The staff here is very protective of us too with Covid-19 and make sure both residents and themselves have the proper protective supplies. They gave me a mask to wear in case I go out into the hall and they always wear theirs. There’s also always something to do with the different activities even if it’s in the room which is great since I like to stay busy. Overall, I have really liked my stay.
My husband was a patient at Heartland Canton recently and we had the best experience there. His therapists Sharon and Henry were great! You could tell they truly cared about his progress. The entire staff in general was very good. Everyone was kind and helpful. I was there every single day with him and the staff made sure I felt just as welcome and He did. He had a few different nurses who were all very good, our favorite was Athena. His CNA Theresa always went above and beyond to make sure he was taken care of. Heartland Canton is an excellent facility in all aspects; from the therapy down to the food, it was all great! I am very happy we made the choice to go there.
I’ve worked for Heartland- Canton for almost 2.5 years now and started off as an Intern. Liked it so much I became part of the team! I love that Heartland offers a wide variety of recreational groups for the residents to enjoy like bingo/games, arts and crafts, religious services, movies, and more. There’s something for every resident to do and enjoy when they aren’t in therapy. Overall, great place to work and it’s always great to see our residents happy and enjoying their time while staying at our facility.
This is a highly recommended center. I had a very pleasant experience. Mom had to go there during the CORONA lock-down. The Staff was great calling me multiple times to see what can be done to make her stay pleasant. They set a weekly time so they can call me on Facetime so I can see her. They communicate with me almost every week. According to mom, the place is very clean and the food is good.
I am the dispatcher at LA Medical Transportation. I wanted to leave a review, because we are truly impressed on the level of service Heartland of Canton provides compared to other facilities. We provide transportation for many facilities in Wayne county, and only few can compare to Canton. Most of the people we provide transportation from Heartland of Canton would share positive stories on their treatment in Heartland of Canton. Not only that, we love working with their staff. They are professional and helpful.
The staff were very nice, and a personal commendation for Delilah, our Social Worker, who went well above & beyond to ensure my dads discharge & transition back to his Assisted Living facility was cared for. She cared for each & every detail as if it were her own family member. We truly appreciate her and the hard working staff there. Its not an easy job by any means, so kudos to Heartland Staff, especially those attending to my dad on the 2nd floor!

I have been employed by Heartland of Canton for the last 3 years. I am proud to be a part of a team who genuinely cares about the success of our patients. Every patient that enters our building is different. Our team wastes no time in personalizing a plan for each and every one, to ensure they reach their goals before they return back to the community. I have witnessed so many amazing success stories in the last few years including one of my own family members. It is truly incredible to witness our team at work and watch the patients hit their goals.
My experience with Heartland of Canton was all positives. I wish people would recognize that no place is always going to be perfect and there will always be opportunities for improvement. This does not make a facility a bad place. I have never seen a facility where the administrative staff are so attentive to patients and families. The administrator, director of nursing and director of rehab are team players and definitely willing to lead by example. I want to express my gratitude for every discipline of care in the facility. From interactions with everyone from the front desk, admissions, CNA’s, nurses, nutrition/dietary, housekeeping/maintenance, therapy, billing office, case manager, and social workers; your hard work does not go unnoticed and the things you do are all helpful. Know that if you have a loved one go to this facility that they are in good hands.
The Canton community is happy to have Heartland Healthcare in our community. And we are equally as happy to have heartland actively engaged with our Chamber of Commerce as they continue to give back to the community at large. On a personal note, I have had family and friends as patients in this facility and the care has always been top notch. Way to go Heartland!!!! You get 6 stars from me. :-)
Update:Immediately after this review I was contacted and and shown much empathy. I appreciate the time they took out to contact me and rectify the situation. A big thank you to the staff that assisted my father and myself even after his release. I still get calls checking on my Father which doesn’t go unnoticed. Thank you!During COVID 19 it was my expectation and hope that my loved one was treated with the utmost respect especially since we are unable to go in and see him, physically. On two occasions my father has had his things given to other residents/patients in the facility. The first time-okay-mistakes happen and nothing was missing. Most recently on 10/27-a nurse gave another resident my Fathers entire bag of things so they went out to replace it only to have one thing missing because the store didnt have it . They literally tell my father The other person (they gave his stuff to) liked it also. The Supervisor on duty -Sarah (The only Sarah in the building as she says so nastily) apologized in a pretty dry tone and said she would go out and get it herself only to find the ONE gas station she went to didnt have it. A nurse tells my father today that he will have it tomorrow (2 days after). So it takes someone less than an hour to give his stuff away and then he has to wait 2 days??? Sarah tells me with an attitude she wasnt driving to 3 or 4 different locations to look for it (although there are stores and gas stations very close next to each other). My family and I travel 30 mins to make sure our Dad gets what he wants and the nonchalant and nasty attitude I was given by Sarah was disturbing. She didnt even stop in to see my dad and offer an apology. Awful.
If you love your family members don’t bring them here at all. We have the worst experience in our life. My mom is here after her stage 4 cancer surgery and she really needs good care. But if you bring your family member here that means you don’t love them. Nothing is on time at all. They have some priority people here who gets the service on time. Most of there nurses are rude. Michigan State must lookup this place closely. Most of there reviews are from there employees and all of them said this place is like heaven tothem because they don’t work much as regular nurse work. Every shift pushing there work to other shift so none of the shift will give you the good service. Even I complaint to one of there head but they always try to protect there nurses. So it’s a waste of time to talk with anybody here. Keep your loved ones elsewhere don’t bring them here.
This is my husbands second time at Heartland Health Care Center in Canton. I can not express enough of how happy I am with the help he has received from OT and PT. As well as the customer service we get from every single person who works for this establishment. From the greeting at the front desk to all the people in the back office who make sure that things are taken care of in the right manner. To the Nurse (Micheal) upstairs on the second floor who treat my husband with respect and takes the time to explain or even to just give an ear on how he is feeling. To the aides who make sure he is comfortable and give a small gesture of encouraging and positive words in the morning to help him start his day. I am thankful to each person who has been involved with care and day to day basis. My husband feels safe and cared for and I also have to say that the activities directors are amazing with the patients as well as my husband and to the ladies in Housekeeping and the Kitchen thank you for always keeping his room in order and food hot on the plate and making sure he is on the Red Napkin Program. I give Heartland 5 Stars because of the service and quality of care they have given my husband and I thank each one for the time they give to him and others each day.
My Mother lives here and is so well cared for that I do not worry (not even during Covid-19). The staff is wonderful. The maintenance staff keeps the building spotless. I cannot say enough about the Business Manager; she is always so helpful. She also walks the floors and understands there are families here. My Mother is in good hands as are all the residents.
Heartland Health Care staff wears their heart on their sleeves. I have had many interaction with the staff members, over the years, from Heartland and it is always a pleasure having them around. I work the Canton Club 55+ and Heartland is always willing to help out at the senior center. I have had the pleasure to work alongside staff from Heartland on numerous occasions an they are truly outstanding. I work for Canton Townships Senior Center-Club 55+ and Heartland has helped us in many ways such as:*Filling in for events when they have fallen through.*Donating a new scooter for us as our original one we had for years was no longer able to be serviced, and it is used and needed by several individuals.*Sponsoring events so we can provide our Seniors with activities.I have also attended some of the events that Heartland has provided and again nothing but the best. David Liebrecht has found the right career. He goes above and beyond always and truly cares and it shows. Its a pleasure to work alongside him to provide nothing but the best for our respective members.
I am a long term care resident and have been living here for almost 2 years now. There are a lot of nice people who work here and they always have fun activities to go to. I’ve made a lot of good friends since coming to heartland. My favorite groups are knitting and bingo but the activity girls always bring me puzzles and what not to work on in the room too so I can stay busy. If I wasn’t here, I’d be at home all by myself. I am very thankful I was able to come and live here.
Heartland Canton has a really compassionate and attentive care team. From the minute you walk in the door, you are greeted by a very friendly receptionist and each face you encounter there is warm and smiling. I work at Waltonwood Cherry Hill and quite a few of our resident family members pay a visit to Heartland when they are in need of rehab. I also hear very positive reviews from their experience at Heartland Canton and know that they are well cared for there. I wouldnt hesitate to send a loved one there for care.

My step dad was recently at Heartland Canton, he was there for 2 .5 months. He suffered a severe injury and needed short term rehabilitation. This was all very new for him and my family. We chose Heartland because we had heard great things about it; My mother and I are so happy that we chose it. The therapy he received there was exceptional and he made more progress than we could have hoped for. The staff was always so nice and caring towards my step father.They treated him with respect and made sure he was always okay. Facilities like these often get a bad reputation, but trust me when I say Heartland Canton is the best place for your loved one to the receive the care they need. From the day we arrived, to the day we left, we always felt well taken care of!
Working with David Liebricht, Business Development Specialist, has revealed to me how special Heartland Health Care Canton is. Hi name Shemika, I heard about Heartland Health Care Center-Canton through my husband who works for an non-emergency transportation company. There was availability and we decided it was the right place for our family as we reside in Belleville. I can confidentely say that the care is A+. The staff truly cares and have never made me feel like a burden which was very hard for me as I’ve lived independently my entire life. I had two surgeries on my back and it’s required skilled nursing and rehabilitation to help me recover. I would high recommend their services. The building is clean and friendly and definitely worth a look. I want to thank you Tia, Irena, and the entire staff for their help.
I recently toured the facility during an open house, I work as the Director of the Plymouth Community Council on Aging. I was impressed with the facility and the staff, the facility was very clean and very welcoming. I really thought the new idea of sending a meal home with a patient that has been discharged was a great idea. My mother was at your facility a few years ago, the rehap she received was very good. I also want to say I have been working with David Liebrecht Business Development Specialist and he is a great asset!

I started as an Activities Assistant, became a Recreation Director, and currently I am the Assistant Administrator at our Canton facility. Heartland is one of the go to places in Canton for our Residents to go for their therapy! They take good care of them and keep in contact with our Nurse so we can prepare for their return at their home/community. David Liebrecht is amazing and comes to our community, Glen Abbey Assisted Living & Memory Care, and participates in activities with our residents and visits with the ones that have been in Heartlands care. We enjoy working with the team at Heartland Canton!
My dad was here after covid started. Great staff and care. Everyone was so friendly and attentive and professional for the most part. But the food....my god I have no idea what they are thinking. Why feed such unhealthy stuff to old people trying to get healthy and strong? I honestly do not understand what the nutritionist and the kitchen staff are doing in that place. Even 2 staff members mentioned they wouldnt eat that stuff.
It was a rocky first day but once we communicated with management things turned around and became very good. Renee the Administrator is excellent and handles problems very fast. My therapists are good. My nurse Bill is a great guy we have a lot of fun together. I have no complaints. Tara in Admissions is excellent, she took care of everything in a timely manner and always has a smile.
My name is Nicole and I’ve been with HRC ManorCare for over six years.Since my start at Heartland I was a unit manager and now our director of nursing.I can’t say enough good things about our team! We care about each other and work diligently each day to care for our patients.We just completed our annual state survey (2019) and scored extremely well. Our Medicare star rating is a 5 STAR. I couldn’t be more proud of our team!My favorite part of my career at Heartland is working with both patients and our staff. Watching those who are new and working with them to train and impact their lives is challenging and fulfilling.Additionally, the daily stories of patients who have overcome injury, illness, and procedures are nothing short of inspiring.
Post hospital stay was in for Phyiscal and occupation therapy. Excellent care and serviced. Nursing care was outstanding, must understand they are working hard and there more residences than staff.Therapy was patient, motivational, and pushed to your personal limits and safety both in facility and post discharge safety practices. Foodservice was good, sometimes items were on the tray that were not desired. But, they are trying to provide a balanced diet.I have been. In multiple rehab/nursing homes for my parents, gfriends s,nd myself. I do see the difference in all of them, good bad and between.Thank you, Bob T
My father was recently at Heartland Canton. It was my first time exploring rehab options but the Admissions team at Heartland walked me through every step. He was well taken care. I never worried about him in their hands. He walked out of the facility within 2 weeks. I would highly recommend this facility to any of my loved ones. Thank you Heartland Canton!
I’ve worked for Heartland in Canton for over 16 years. I started out as part time floor nurse and have been working full time as a MDS nurse for over 14 years. I love my job and I love the people I work with. It’s not just a place of employment they are FAMILY! It’s a great place for rehab and you will leave knowing they did everything for you!
Ok 1st impression of this place is bad. Wife arrived from hospital for rehab, still need assist to stand, let alone walk to the bathroom. We been in her room for over 1 hour and not one staff has come by to let use know what is the process here. We rang the call light, they came by an said be back, NEVER came back. I went to the nursing station, they acted as if I was bothering them an kinda just brushed me off. Dirty smelling sheets on the floor when she arrived and still her almost 2 hours later. The feeling is like a nursing home.
I have 6 pages of complaints against Heartland of Canton.For my wifes stay there, we informed them that she is allergic to lactose, rice, pork and turkey. Also that she has no teeth and can only eat soft foods. Guess what? They continued to feed her the items she is allergic to, served crispy hard foods that she cant eat, and it was cold! Coffee was cold, soft drinks and water HOT. They really have it backwards! I had to bring her food from home so that she didnt starve while there. They never replaced the food she couldnt eat, even when they said they would.Nurses changed the schedule of pain medication to suit their shift wishes, not the doctors orders or didnt deliver them due to shift change. The other shift thought that she was already taken care of. NOT!Nurses and aids take 30 to 45 minutes to answer call bell. I had to take my wife to the bathroom myself one day and no one ever showed up to answer the call. She couldnt wait anymore than an hour to go to the bathroom!They still owe us a replacement check for $200.00. My fault, I lost the check behind my file cabinet and didnt find it until it was expired. I called their business office. No problem, just send me the check and I will have it replaced for you. Three months later, nothing. No phone call, no letter, no check.I will update this when and if I get the check.Recommend? No. Dont send your loved one here. There seems to be only one or two nurses there that really care for their patients. Its just a job for everybody else, where they can get away with NOT doing their job. And if you really want a financial headache, then go ahead, use their services.Like another poster said, the positive posts here are probably fake because we never met anyone there that was happy with the service or care provided. Guess we should contact the state licensing agency.
I had a family member at Heartland and it couldnt of been a nicer place to be. Clean and sanitary conditions. The staff was very nice and they addressed all concerns and needs for the family. I would refer anyone who needs short or long term care to Heartland of Canton.

IRRITATED! Finally whitmere allows visits if conditions are met after over a year of no contact with loved ones, yet I still haven’t got to visit because there is someone that always has Covid. Like clockwork I get that automated computer phone call notifying me of a new COVID. Maybe staff should be restricted from leaving the facility so they would stop bringing in the virus! Or is this the way to make sure there is no accountability to the patients if outside can’t check their well being? I have many other concerns, but this is down right torture to the patients. They can’t even leave their room for activities or dining, except for the patient that walks the halls and enters rooms to disrupt other patients. Even a dog is let out of the cage occasionally!,
I worked here for a short 2 months. I really enjoyed the facility, but it just didnt work out for me. I tried as hard as I could to stick it out. I was suppose to be a full time employee but was called off at least 2 or 3 or the shifts out of my 8 day work period. I struggled finding help from most of the aides. There were a few that were very rude to me and made me extremely uncomfortable coming to work after one told me to stop asking her for help. As an AIDE you can not do this work alone. I felt like I couldnt give my 100% to my patients without the help of my colleagues. It was too much without help. I am thankful for the physical therapy team because without them I would have not made it as long as I did.
The PT and OT program was wonderful and the nursing staff is great! facility is very clean and the house keeping staff does a wonderful job keeping it that way. The maitinance team is great too, they fixed my wheelchair within 20 minutes of my request, which is impressive because they are so busy. Also, the recreation department is wonderful and they deserve a special shout out, the projects they have set up for us were great. All in all its a good place.
Although this is an older nursing home it is clean and kept up.The nursing staff were awesome! They really helped my family! Many thanks to Katrina, the director of nursing and nurse Laura for their dedication to their patients. Thank you for your wonderful care of my Mother.
This is a beautiful facility with wonderful people. Every time I walk in the door I am greeted by Elaine at the front desk who is always smiling. The Social Services department are wonderful. They are all very knowledgeable, very helpful and very kind. There is a beautiful Therapy room with many experienced Therapists. I would highly recommend Heartland Canton.
I am starting my 22nd year working for Heartland. I worked my way up from a staff therapist to a director, to a senior director. Heartland has integrity and has never put me in the wrong direction. I feel like they invest in me as an employee and a therapist. I have been at Canton since the doors opened and have seen it grow. We received shirts for our rehabilitation week celebration this year that says “ your wellness is our mission” and I firmly believe that.

I was recently a patient at this building and i liked it very much. The staff was very caring and always made me feel like a priority. I was worried when i first came in that i would be stuck in my room all day when i wasnt in therapy, but there was always something for me to do. The Recreation girls have multiple activities to do through out the day to help keep us busy. Most importantly my rehab therapists were amazing, they truly cared about my progress and I would not have been able to go home with out them. Excellent care! if i ever have to do rehab again it will be here, no doubt!
My father was at this facility. He lost a lot of weight and I found him one day trying to change his own brief. When I went to discharge him against medical advise to place in a different facility, I waited an hour for the discharge papers and the next day had Adult Protective Services at my door accusing me of kidnapping my father from this facility. I guess the social worker was upset because I wouldnt wait two days for her slow response to wanting my father to get better care. Beware.. if you dont want APS at your door, dont place your loved one in this facility. Truth is, APS should visit this facility and watch the patients try to change their own diapers, because that is exactly what I walked in on my father trying to do because he was in it for HOURS.
Heartland Health Care Center - Canton is one of Canton Townships wonderful community partners. Tanya and the organization are very supportive of our programs for those age 55+ and are always offering to get involved in a number of different ways. All my interactions with the Heartland team have been amazing. Great job!
I transferred to Heartland Canton from the ICU at St. Joseph Mercy Hospital-Ann Arbor after I almost died from a burst abdominal aortic aneurysm while driving home from work. I had gone through six surgeries in seven days, forty units of blood and had a 5% chance of survival. I wanted and needed a good rehab facility. When I got there, I couldnt even sit up in bed without help. I want to thank both Lee (physical therapist) and Nick (occupational therapist) for everything they did for me, as well as the rest of the staff. I did everything that they told me to do and eventually I walked out the front door and went home. It was a lot of hard work but they also made it fun. They are truly life savers! Thanks for everything guys.If you want to get back to a normal life from whatever you suffer from, you need to do everything these guys ask you to do.
This is the worst place Ive ever had the displeasure of visiting. We put my grandma in here for a month following a surgery. She couldnt walk, so she couldnt go to the bathroom on her own, or anything else. When she would push the call light for somebody to come assist her to the bathroom nobody would show up for like an hour- not exaggerating at all. There was one day that they put pants on her that were too tight that she wanted changed because it was hurting the incision from the surgery.. nobody came. I had to change her myself. What are these people getting paid for? Another day another family member had to change her diaper! Absolutely ridiculous. The staff is seen on the phones, when they should be caring for their patients. Its clear that most people working here dont care about their job or their patients at all. Would not recommend at all.
Hello Name is Pam and I have been with the company for more than thirteen years. I have worked in many departments from being a nursing assistant to now working in Medical records. I like working with my coworkers and and working with the patients to absolutely have the best possible stay.
I will try to be brief, How is this place still up and running? My mother had spine surgery and needed extra care post-op, Sadly this place was recommended to us. From the moment she arrived it was one issue after another, Nobody knows what is going on. When speaking with “Managers,” They are clueless as well and actually tell you that!OT pushed her through therapy one day this week, Even though my mom struggled so badly! My mom said she was dizzy, She was very abrupt with her and just kept pushing. The next morning I overheard a conversation that my mom fell early that am, I guess I wasn’t supposed to know that! What??! So, Do you think Therapy was so difficult because of a fall after spine surgery???? Then to make matters worse, OT did not have time to stay with my mom for toileting So she leaves her on the toilet! My mom passed out, Thank the good Lord I was there and standing checking on her outside the door. Excuse after excuse is all I heard for a week! Yesterday I made the decision to move my mother from that hell hole, And what a great decision.People, Don’t send anyone to this place.My mother got a sweet roommate who looked to have end stage dementia, Half of her body was on the floor when I came in that morning. The staff was told by the nurse to put her in a wheelchair and park her down by the nurses station! That is pitiful! Her breakfast came, Nobody had time to feed her. She didn’t have anyone there to see what was happening, Don’t leave your loved ones and think they are being cared for.I was there every day 12 to 14 hours, You would think my moms care would be a bit more on point! All i can say is I wasn’t there for friendships, My mom will get proper care if I have to babysit EVERYTHING you do!I am not done with this, as soon as I get a moment to find the right channels to do some reporting I am doing it. I have documentation, I have names. Stay on it people!I moved my mother to another rehabilitation center, Wellbridge of Brighton. Difference of night and day, NEVER had to address one thing in almost 3 weeks.Ran smoothly from the moment of admission, Sat down with the administrator and she shared with me exactly how things will run. Please do not subject your loved ones to this place that needs to be shut down. I am in the process of doing some reporting, It isn’t over. My mom lost an entire week of healing from this awful place. They couldn’t even handle a urinary tract infection, Really??? Didn’t give a long enough regiment of meds, So it never went a way!!!This is unacceptable folks!!!
Not sure on the care itself but one of the social workers here Tea is NOT very helpful. Have a family member in the facility and she keeps pushing,asking us what our plans are. For this family member once he is discharged. For people who have no idea what to do or what our options are she is not very helpful with getting us any information we need to make a decision for him.
I would recommend this location to anyone of any age. I am 37 and was welcomed with open arms, the rooms are clean, the staff is very friendly and always helpful, the food (in the Bistro) is always hot and served fresh, and the 100% therapy guarantee is true. Overall, I am as prepared as I ever could have thought to go home and continue my recovery...with the start I needed to heal better, to heal faster, and to succeed. I, honestly, have nothing negative to say about this location. If youre looking for an excellent sub-acute rehab facility where you will actually leave better than when you arrived, this is the place for you.
I really enjoy the therapy here. This is my 5th time coming to the Canton Heartland and when I knew I needed more therapy after the hospital I requested to come back here. I’ve never had a bad experience here and I LOVE the different activities. They offer everything you could want to help you have fun in this type of situation. As long as I need rehab, I’ll be coming here.
Never Again! Now that my mom is safely out of Heartland and into another facility, let me start my review. I try to be as honest and straightforward as I can. On May 29th, St. Joseph Hospital transferred my mom to Heartland in Canton. She had been in the hospital for 6 days with a nasty bout of Superfiscial Thrombophlebitis. She was bed ridden with IV antibiotics and very weak. She is in her upper 80s, and my only surviving parent. My first time visiting her at Heartland, I barely recognized this shell of a woman, parked in front of the RN station, in a wheelchair and her head down on a rolling table. Shocked by this pitiful sight, I gasped Mom??? and she lifted her head; confused, disoriented and extremely fatigued. I wheeled her back to her room and helped her into her bed. Her pants were soiled, the pillow she was sitting on in the wheelchair was soiled and she was in very dire need of attention. The RN came into the room within 10 mins and quickly explained she was out there because she had a history of falling and they didnt want her in bed alone in case she fell. So, the next best thing is to force her to sit in a wheelchair and park her out to the RN station and give her a table to rest her head on for a couple of hours. Really guys? Needless to say, this was NOT a good first impression. As if it couldnt get any worse, 2 male staff members came into the room and closed her divider curtain while me and my 12 yr old son stood there and listened to her distressed grunts and moans while they stripped her naked and changed her diaper. I was in complete shock. Why arent there female staff members to assist the female patients????? Fast forward 2 weeks later.... I have gone there every day and my list of quirks include: 1) It took 4 days (and 3 separate times at the RN desk) for me to ask them to please change the battery in the wall clock in her room. She was already confused and the only time in her room was 7 hours off. SEVEN HOURS OFF. Completely unacceptable because every single time I asked , I was told it would be done. 2) The time it took for anyone to come once her red alert button was pushed.... since she had to use oxygen, she had a floor machine as well as a tank on the back of her wheelchair. When I came to see her, she was desperate to get out of bed into the wheelchair or vice versa because her back was killing her. Because shes tethered to this oxygen tube and we werent allowed to switch from the floor machine to the tank, it took on average 15-30 mins just to get this simple job done. Very aggravated by the lack of staff and/or concern for patients who press the red alert button and get NO ONE. Had it been something more serious, the patient would most likely die before being attended to. 3) Long fingernails... On multiple occasions, my mom was scratched by staff members who changed her diaper because of their long fingernails. I work in a profession where I am required to keep my nails short because I touch people and long fingernails should not be accepted in this line of work... PERIOD. If you want to keep your fingernails long, then maybe you should find another profession where you are not hurting patients. 4) My mother was absolutely mortified when a MALE STAFF MEMBER took her and gave her a shower the other day. She was very distraught and embarrassed by this and I personally think there should be a strict policy on FEMALE STAFF ASSIST/SHOWER FEMALE PATIENTS and MALE STAFF ASSIST/SHOWER MALE PATIENTS. This was the last straw. We pulled her out as soon as we could. She is still recovering but now we have a peace of mind and know she is much happier. To be fair, the good points were: The OTs and PTs there that worked with her. Its a 5 minute drive from my house. Heartland, please take this and all the other critical reviews seriously, when you have so many strikes against you, there is a real problem and it must be addressed.
I was discharged yesterday from this facility. My departure typified my entire stay. They handed me some papers ( my discharge instructions) did not review them and asked me to sign. I did sign, but I was careful to write down the fact that the instructions had not been reviewed.The nursing assistants are terrible. It took 5 days to get a wash cloth and a hand towel. I finally stood, with my walker, at the nurses desk and asked everyone who went by if they knew where I could get the towels. After 15 minutes the nurse behind the desk opened a drawer (didnt even have to stand up) and handed me the towels.I accidentally spilled my juice and asked for help. The folded the sheet over the wet sheet and left it. The tray was left dripping juice and I did not get a replacement breakfast.This is just a couple examples of the poor care I received from the aids.It also proved to be almost impossible to get my prescribed pain medicine. One day, after waiting 3 hours and numerous calls to the nurse, I crawled on the floor to get the medicine. A couple of the nurses were outstanding, Teh and Heather on the night shift. Everyone else was there just killing time until they could leave.The food is horrendous. If you plan on putting your loved one in this facility, plan on bringing in meals. The food there is uneatable.The case worker Pat is also terrible. Did not assist me or bother to give me an explanation. If you can believe it, I had Blue Cross trying to approve an extra week and Pat would not fill out the paperwork. I still dont understand why they didnt want to accept my money. I guess it was a lot of work to fill out the paperwork.Pleas think carefully before you place a loved one in this facility.
My parents and uncle dropped my 97 year old grandmother off here this afternoon before 3pm. They couldn’t get ahold of the correct number for her for 5 hours. No one at the desk would answer either. When they finally got ahold of her, she told them the center left her sitting in her wheelchair until 7pm with no assistance to use the restroom, and no food. When they finally fed her it was only a tuna fish sandwich. They then left her alone to get to bed herself.She is in the center because she needs assistance doing these things. Because she needs to be rehabilitated and assisted in getting her strength back. At this point we are all very concerned for her safety. If they can’t feed her and check on her, how are they doing with her meds? How will they know if she has an emergency? She is 97 years old with a variety of health issues, including hearing disabilities. She’s weak because of difficulties in doing things for herself like eating.These are issues they should be well aware of.
If I could give negative stars I would. My father has been here for 30 hours and this place is a terrible joke! My Dad has an open wound on his foot that needs packing, his bandage wrap fell off around 1700 we told the nurse who through her hands up in the air and yelled Im out, Im out, then his night time nurse came in at 1930 and asked what our concerns were, we tell her, her reply is that she has to go see her other patients before she can help. Then advises me that Michigan law requires every patient to be checked on every 2 hours and walk out of the room. After making a few very rude and uncalled for remarks, telling me to pull it together and that I need to be prepared for anything because we all gotta sit in that front row girl!!!! WTH!!!! So we wait it is now 2230 and still counting and no one has been in our room to give meds or pack his wound or even to just check on him, as per Michigan law so she says. I call the nurses station to ask for a unit manager or charge nurse and low and behold there isnt one on duty and there will not be one on duty until between 0800 and 1000 tomorrow. All this while the lpns and the aides are out in the hallway gossiping and laughing and being as loud as can be. This place is a complete joke, the only staff members that showed any caring or compassion were the OT/PT staff. The nurses and aides are useless, and they do not care about anything but their pay checks, which they are constantly complaining about. If you love your loved ones DO NOT, I repeat DO NOT bring them to this place. The hospital liaison lied to us about the staffing, she is completely misinformed about this place, and its a shame. For people that have never been in a situation where they have to pick a rehab facility, the lies and misinformation about this facility are overwhelming, had been told the truth we never would have chosen this place. Oh, and on the tour they take you to the unit that the patients need the least amount if help where everyone is mobile, DO NOT be fooled. The private rooms are big, but the wallpaper is peeling and the walls are dirty the furniture is old and uncomfortable, very out dated - not like we were shown via tour. In the span for the 30 hours we have been here 2 emails were sent to the facility managers and nothing has been addressed. Its a total shame.
My husband, Mark, recovering from an anoxic brain injury due to cardiac arrest, was unable to sit or stand unassisted when he arrived at Heartland. The therapists strategically worked together (Sharon - OT, Charlyne - PT, Sarah - Speech) to ensure Marks best recovery. I have never seen such an intelligent, hardworking, and dedicated team of therapists. My husband is walking, talking and living his best life!God bless the Heartland therapy team! And God bless all of the extra special nurses and nurse aides that carried us and loved us through a challenging journey!
This place is the worst. My sister is there. They have not given my sister a bath in a week. They give a diabetic foods loaded with sugar and she has to beg for her pain meds. If you love your family members or yourself stay away from Canton Heartland. I called to ask who owned this company to be hung up on, called back spoke to Beatrice who would not give me anything other than it was in Toledo when I asked who was in charge they said they were on call (Caroline) but would not give me the number. It is a disgusting how they treat their patients. I will follow up with the corporate and every medical area to make them do what is right.
The physician treated vasculitis (thank you Livonia dermatology with a proper diagnosis) with benadryl for 6 days because they thought is was just a rash. Also found my mother when I arrived in the morning without oxygen when it was ordered. This was my first experience with a rehab and after 4 months of hospitals and 2 different rehabs this is not the place for a loved one. St. Marys will recommend this facility and for the life of me I cant figure out why. The staff was incompetent and negligent. Again they treated vasculitis (inflammation of your blood vessels resulting in organ and tissue damage) with benadryl for days. Keep checking and visiting facilities. This is not the place for someone you care about.
This place is terrible!! they do not care about their patients. DO NOT SEND YOUR LOVED ONE HERE! The “lpn nurses” are rude and don’t care about any of your concerns. Everyone is lazy and complains of u ask them to do anything. This place should be under investigation by the state all they do is charge the insurance and provide no to little care! I had a one of the cena’s tell me everyone is lazy here and no one does their job. I had some concerns about my loved one that I addressed to the “nurse” and she said he’s fine.... well the same day he got admitted into the hospital with a brewing infection. This place nearly killed him!

Elderly dad was there for 3 weeks post hospital stay for physical rehab.Physical therapy - great!Facilitys cleanliness and overall atmosphere - great.Food, day-to-day service - good.Communication - poor.EXTREME frustration with their lack of communication with contact person/family and their follow-up when I did manage to communicate with someone.Despite those problems my father greatly benefited from the care he received there and the aftercare provided by the home care company which they arranged.
After 4 weeks.... PT and OT staff are v good (4 stars) Nursing and medical care is poor (1 star). We were on the 2nd floor that is set up for patients that need more care. Overall takeaway is that I dont feel my mom was well cared for and in some instances didnt feel she was safe.Changed the pain meds without notifying family and dont monitor pain levels.Poor communication. No single point of contact for a patients care, multiple people assess but no communication or coordination between them or with the family. You pretty much need to be there 24/7 and sort things out yourself.Multiple times have found my mom had the oxygen cannule in her nose but the staff did NOT hook it up to the oxygen machine!!! RISK.Let prescription expire and then have to fax to the dr on sat night, delaying meds. Poor process creates crisis and stress for staff, patient and family. Meds changed without informing family or set up differently than agreed in discussion with nurse practioner and Dr. Poor process all around.No orientation on meal options or that you can file a written complaintSome of the staff are v good but many lack compassion or motivation to give patients good care. When u present them with an issue you are likely to hear excuses and I wasnt working that shift vs plan to fix. Appalling attitudes at times. Spoke with Renee and left msg for nicole. Little or no response or improvement.Need to post patients PT and OT schedule in the room. Aides and nurses are completely unaware and we were told the aides should be getting her ready for OT.Probably a ok if u are independent and just need rehab, but do not recommend for elderly patients with some memory issues, dementia, etc.and or anyone that needs more than very basic medical care.
This place is horrible. Please please please do not bring your family members here. My grandmother asked for help and her medication over an hour ago. I also asked for someone to help her and the employees are being very rude and keep telling me that they will come back to assist and then they do not come back. I need to know why my grandma is being mistreated here. I am shocked and my grandma is so uncomfortable. She keeps crying because the nurses areIgnoring her pain. This place is disgusting and the staff is too busy texting in the hallway to care about the patients. I heard a man down the hallway scream in pain and a staff member told him he needs to stop complaining and deal with it. I am going to have nightmares tonight knowing my grandma is here all alone.
My elderly parent was there for just under 2 weeks a few months ago. The facility is clean, staff is friendly and generally responsive. It is rehab, so patients are expected to do some of the stuff themselves. We visited several times daily. Food was ok. I appreciated what they did and would take my family back again if needed.

My neighbor/adopted mom was taken the Heartland of Canton for re-hab and was there just short of two weeks. She was on morphine for pain caused from cancer in her shoulder and back. One morning instead of giving her 10mg of morphine she was given 100mg. Needless to say she was taken by ambulance to the hospital and almost passed away. How could this happen?? She spent 4 days in the hospital and is now in a different facility doing well.
I would not recommend this place to NO ONE I love or hate. My love one called for help and received an 8 hours response. She was on a restricted diet and they were giving her food that she could not eat with her health condition. Too much to complain about this facility. We was so sure she would have been in GOOD hands here...what a disappointment. As a matter of fact, my love one died there and I have proof that she died because of the lack of care that she did not receive. I am sharing my experience with everyone in my building.
I participated in an open house. I felt very welcomed by the staff and the host and leaned about unique initiatives such as weals to go:. I like the facility lay-out with easy access and visible instruction. I perceived a positive energy in all rooms we walked in. I noticed a massive number of hand written complements posted in a wall which gratifies who gives and who receives. Well done.
Rehab is good. IF your family member is not independent, this is not the facility for you. They will wait for up to 15/20 minutes for help. Aides are generally friendly but you are a number. Upon going to the hospital, I got a call 45 minutes AFTER the hospital called. I was already in the emergency room. Communication is extremely poor between family and administration.
Heartland has Amazing PT and OT Therapists! Mamta Peter is a thuro OT therapist always thuro from start to finish. We saw advancement in my fathers right arm ( Who has suffered strokes and has weakness on his right side.) We thank Mamta for the care you have shown in helping him to reach goal. When my father first arrived he had Lee as a PT therapist who jumped right in with a great program of stretching muscles in his leg and hips that had never been stretched since his stroke. We noticed improvement in his steps. His PT therapist was changed after returning from the hospital and Kothai became his PT Therapist. She was kind and understanding teaching him to properly sit and stand, stretching techniques and words of encouragement after him attempting steps. I cannot forget Carlos ( PT Asst) Great guy we thank you for all you did and do everyday Dad always said you were a good guy! I thank each one of you from my heart that the time you each spent with my Dad will not be forgotten and my mom and I cant thank you enough for the help you gave, the smiles you brought, the words of encouragement gave Dad hope and for that we thank you. Any patient would be lucky to have Mamta, Lee and Kothai for therapists and this is why I give 5 Stars.
Horrible care! My mom was there for four days too long!!! Could never get ahold of anyone on the phone to check on her. She called 911 twice to get her out of there!! My brother went there and found her on the floor. No one ever answers the call light, Im not certain she ever ate when she was there except the food I brought for her. I never saw a doctor. The last straw for me was when I went there after work after calling there all day to speak to someone so find her sobbing, wearing the sameClothes she had on the day before but now soiled with something, her oxygen off, the call light in the floor and the phone on the floor as well. Her backside now looks as red as a baboon!!! Im hoping she doesnt get a bedsore!!!! Absolutely terrible!!! If you want your family member to die a slow and horrible death take them here.
Unfortunately...I have to change my review. This place has the WORST OT and PT program (I have worked/been a patient in a few programs). After only 3 weeks they told my 72 yr old mom she has baselined and are discharging her. She came to the facility after surgery for a broken femur. She was completely imobil when she arrived..And has had steadily improved but has had some setbacks with infections and an allergic reaction to medication. She is still wheel chair bound....And is unable to steadily walk with a walker. They are sending her home by herself....( She lives alone)....With out making sure she can go to the bathroom by herself...They havent even worked with her on getting in and out of a car or preparing a simple meal. She is now mentally depressed after being told by the therapist that this is her new base line...(she was fully functional prior to breaking her femur). Who tells that to a patient after only 3 weeks!
Many mean, rude, unprofessional staff. Very negative and disrespectful treatment of the patients. If you complain they will abuse your loved one even more. I am sure the positive reviews are fakes written by the management. My husband was afraid of most of the staff, especially the cnas. The doctors only show up occasionally, and were rarely available by phone by either the RNs or myself. They lied to my face on many occasions and blamed any problems on my husband, a helpless man unable to care for himself. He was often left unchanged and was afraid to ask for help for fear of verbal abuse. When he complained of back pain they called him a drug addict, instead of checking and testing for reasons why he was in so much pain.As it turns out he had a cyst on his spinal column.This was discovered after I had him sent to emergency at the hospital. DO NOT SEND YOUR LOVED ONE THERE!
DO NOT COME HERE!!!!!!!!I am disgusted with this place and cannot believe how the staff neglected my grandmother . Now she is fighting for her life because this place didn’t do their jobs. I don’t understand how this place is still running ...all the reviews on here that are “positive” are all from co workers that work there and were probably told to write a review to make it seem as if this place is the right place to come to.WRONG.If you love your loved ones and want them to really get better and have great quality care, DO NOT even give this place a consideration .
My experience at Heartland health careAdult rehabilitation center and senior living center was caring and welcoming. I felt like I was at home and the food wasgreat !! Had an interview here. They kept me waiting in the lobby for over an hour. They claimed Myrtle couldnt be found. The Head Janitor interviewed me. We were rudely interrupted. Just downright rude and unprofessional. Time I wasted there I could have been picking up my husbands seizure medication. You have to wonder if theyre treating prospective employees like this whats stopping them from neglecting their residents.
So1 gal from their staff called me & I was driving at the time, so I asked her if shed text her name and # to my phone.She said she would.But Ive never received anything.Theres been an issue with getting the staff to help her with her food tray, as her shoulders and arms fingers....are not working as they should.NO ONE EVER ANSWERS THE PHONE.GETTING VERY AGGRAVATED
Please do not leave your love one there the nursing care is awful they sent my husband to the hospital without notifing me, went to visit and seen that he was not in his room i ask the so call nurse where is my husband and she said OH we sent him to the hospital I ask why was i not notified she said I was busy and forgot . Ithink she should be fired but i bet she wont, And as for the Dr, on staff my husband never saw him,So think twice about placing your love one in that hole, As for my husband he is having theraphy at home with a therapist and doing great,
Couldnt believe the way they treated my love one. She ended up passing away and I 100% think that this place & their terrible care played a part in it. Please dont send your loved one here. Nurses in phones even though call lights are on, cussing at patients, acting like their needs are an inconvenience to them as if its not there jobs. Terrible.
My mother was there recovering from a broken back, hip, heel and another bone in her foot. While there they never answered their call light one time. She even tried yelling for help after 30 minutes of no help. When yelling didnt help she actually had to call them on the phone. When she asked for she pain medicine after 45 min of nobody coming back she had to yell again because at this point she had tears flowing down her cheeks because of her pain. This was all within 48 hrs because she was moved to another facility.
My brother has been there 2 times. The nurses nor the aids are not very nice. My brother will need help often throughout the day and when you put the nurses call light on, you will wait any where from 45 min or longer before the come see what you need. Then after they come into your room the nurses WIlsay OK we will be back and its hours later.
TERRIBLE place DO NOT LEAVE YOUR FAMILY!!!!! My father was here for 3 hours tops which in this time he had fallen and hit his head when hes on a Blood thinner and had a fall risk band on his wrist. They DIDNT contact us and even LIED when we were attempting to find him. DO NOT LEAVE ANYONE YOU CARE ABOUT HERE. After the hospital discharged him we had him moved someplace else.
It was a godsend for me. I went there unable to walk due to a break in my back. From day one every way so kind. The love I felt has changed my whole outlook on life. I had never been nor wanted to be an invalid and Im not Thanks to the staff there.. I love those guys. Id recommend it to anyone who wants to be able to be all that they can be.
My mom was there for 2 weeks and she receive the worst service ever , it’s very bad my mom didn’t get the care she needs she would turn the light for a nurse and it would take them hours to respondI had to move her someone else, so please people do not take your love ones there
I can see the therapist fantastic the nurses are good but thats it the food is horrible trying to get someone to help you is almost impossible I have asked 5 days interroll if not more because they feed you from 11 two six it night for dinner I am a patient where I must eat some type of snack a half a sandwich anything they have done it once in two weeks since I have been here that I did not have to ask for Im supposed to be drinking Ensure Im lucky if I get it once a once or twice a week I dont know if its the the nurses aides are who it is or the kitchen that does not know how to do their job the food is the worst I have ever eaten in my life I have never had a hot meal here yet then I hear that my roommate is going to a private room I have been on a list for a private for two weeks they say the other part of the hospital is good they make sure you have your hot meals and make sure you have everything you ever needed for some odd reason this part that Im in the ignored Ive Been Told and retold The dietitian in the kitchen its been told I have personally talked to dietitian and it does not help they do what they want to do and no matter what you say or do it doesnt help like I said the therapy here is fantastic its helping me a hundred percent the nurses make sure I get my medication on time but thats about it I might as well be on my own then in here
Dont let the attractive facilities fool you. I would not recommend this facility. Administration and patient care staff is unresponsive to patient needs and requests. Clearly the problems start with Administration and Management and filter down to customer facing staff. Communication between the different service providers is extremely poor. The result is poor if not dangerous patient treatment.
My stay at the Center was great, I was there for the rehab and the aids and nurses were great when I needed help they were there . My O.T and P.T. , Jennifer and Kumar were there to push when I was saying I cant do it , they kept telling me I could , and I did it. I recommend this place as A+.
Communication is very poor between the departments. Waiting to go to the bathroom is for ever. Now I am waiting for the Social Worker to call back. It seems that NO ONE knows how to have a letter written by a Dr. so I can have a Power of Attorney over my Mother while she is there.
DONt LET THE NICELY DECORATED FACILITY FOOL YOU. I WOULDNT WANT MYSELF OR A FAMILY MEMBER STAY HERE. THE LPNS AND SOME CNAS ARE THE MOST LAZY, IRRESPONSIBLE, DISRESPECTFUL AND UNPROFESSIONAL PEOPLE I HAVE EVER MET. NOT ONLY ARE THEY THESE THINGS TO THE RESIDENTS, BUT THEY ARE ALSO LIKE THAT WITH THE HARD WORKING EMPLOYEES. I CANT SAY ENOUGH BAD THINGS ABOUT THIS PLACE. I WISH THEY CAN GO OUT OF BUISNESS SO THEY CAN STOP CHARGING PATIENTS FOR THEIR TERRIBLE SERVICES. PLEASE STAY AWAY FROM THIS PLACE. THEY DONT DESERVE TO TAKE ANYONES MONEY. I WISH THERE WAS AN OPTION TO GIVE THEM ZERO STARS.

2022-02-21I have worked as an occupational therapist assistant with this company for 25 years. I have been in the Canton building 8 years, which of course includes the beginning of the pandemic. I only can say that the teamwork of this building is amazing. Housekeeping is so diligent about keeping the building clean, the dieticians and kitchen do their best to accommodate patients needs, nurses aides and the nurses are amazing as they will come in on their day off or cover another shift if we are short handed. I also feel we have two of the best social workers. They really get some complicated cases, and they work so hard together to make sure the patient is safe when discharging.Lastly, I can’t say enough about the therapy department. No matter what the circumstances, we work hard to make sure the patient becomes as self sufficient as possible! We work with all kinds of personalities and ailments, and I truly believe our success is because the patients know we generally care about them.I am proud to have been with this company for all of these years and would recommend our services.

2022-02-09My mother-in-law is receiving excellent care at ProMedica. Communication with personnel has been really good. I greatly appreciate flexible visiting hours and experienced and capable staff. The admission process was efficient and all the staff was warm and welcoming. I am so relieved that my mother-in-law is in good hands. Thank you to everyone at ProMedica!

2022-01-19This place is absolutely horrible!!! My mom was sent there for rehab to get stronger to come home. My first encounter when I reached the 2nd floor to visit my mother was the smell of urine. I was appalled. Anytime I called to get an update on mom, the person that would were rude and no one would ever come to the phone. My mom wasn’t being taken care of because the 5 days my family couldn’t see her because of a covid outbreak. She was never fed, my father and I would come up and encourage her to eat and help her when we were allowed. We begged them to go in help her but they wouldn’t let us. We know they were short staffed. We are both vaccinated and I work in a hospital so I know the correct precautions. She got so weak, she stopped answering her phone and got severely depressed. She also ended up getting a bedsore from not being moved. So I know she wasn’t being checked on in a timely manner nor looked after. It was like she was forgotten yet my father and I fought so hard to go in and help her. The wound got infected because she wasn’t being changed enough based on the type of infection that was found in her wound. They sent her to the hospital but she was so malnourished that when they ended up doing a procedure to help with the wound care, she passed away. I felt that people should be warned about this place because I am hearing the same situation from other people. I never give bad reviews but I firmly believe they did more harm than good and my family and I put our trust in them. Instead of bringing my mother home, we planned her funeral.please don’t bring your loved ones here.

2022-03-07The nurses and CNA’s have been so kind attentive to my needs. Everyone has been so patient when I had requests. Therapy has been great! They are very good at their job and have made sure to challenge me so I can meet my goals. I would recommend ProMedica of Canton.

2022-06-20Trust this review when I say look elsewhere! My mom went here doing fairly well on her own, walking/talking etc. She only needed basic physical therapy rehab but after just 1 week she declined real quick. She had to be transferred to the hospital and sadly is now in hospice care. NOTE: She was fine going in there but from the very conflicting nurses stories and many other issues we had we believe there was serious neglect going on. Dont give me the customer service phone number to call, I wont use it. Sadly this seems to be a common problem, lack of staff only goes so far. My hope is someone will read this and go anywhere than here if you value your loved one.

2022-01-31My experience at ProMedica Canton has been exceptional. The nursing staff and the therapists encourage and motivate me to meet my goals. This is my third stay here and I would not go anywhere else.

2022-05-11This facility currently has a covid out break now my mother has Covid. They do not care that she has no place safe to go, they are discharging her as a covid positive person into the community and expecting that we bring her to our home. They had a mothers day celebration where it most likely got the covid since they had people sitting together and eating. The manager lied to me saying they were seated 6 apart and they were not. They told me my mother had PT today and she has not. They told me she would be discharged on Friday and she is told she is being discharged tomorrow at 11 and I have received no call. Their infection control is poor. I do not recommend. No one has reviewed her medication or changes, etc. She has no idea who her nurse is on a daily basis. Unacceptable.

2022-06-22If you don’t care about your loved ones care, send them to this place. The attention given to the patients are much to be desired. In 6 days of my loved one arrival, he was rushed to the hospital 3 times with infections that could of been prevented if a trained and or conscientious nurse was looking after him. The last infection, diagnosed is sepsis, could have killed him if my niece who is in the medical field hadn’t insisted he be transferred to the hospital again. The nurse at this establishment told her, “she was over reacting” but she still insisted and he was transported finally to the hospital with sepsis. Worst place ever.

Hours for staff to return call light buttons, atrocious customer service, rude unprofessional saff. Staff and doctors more than likely took online courses with ZERO CLINICAL EXPERIENCE HOW TO CARE FOR HUMANS! 3rd world, neolithic countries are ran better. You all should be ashamed of yourself! Simply pathetic.
